Students entering the 7th grade or by 12 years of age, which ever comes first, will require one dose of Meningococcal conjugate vaccine (MCV).
In addition, a booster dose of Tdap (tetanus, diphtheria, and pertussis) vaccine is required for individuals who have not previously received it and are entering the 7th grade or by 12 years of age.
You will have 30 calendar days from the first day of school to present the required up-to date immunization record for your student. If proof of vaccination is not provided by the end of the 30 calendar days, your child will not be permitted to attend school until we receive proof of immunization process.
A copy of your child's immunization/shot record is acceptable proof.
